Does a retaining wall need engineering?

Above a certain height it does, and the threshold is set by your municipality. Low garden walls generally do not, but anything holding back a meaningful slope, or supporting a driveway or structure above, should be engineered and prepared for permit review.

Why do retaining walls fail or lean over time?

Almost always water and drainage — pressure builds up behind a wall that cannot drain, and it pushes the wall out. Proper drainage, backfill and a sound base are what keep it standing, which is why the part behind the wall matters as much as the face.

What goes behind a retaining wall besides the blocks?

A compacted base, free-draining backfill and drainage to relieve water pressure, and reinforcement where the height calls for it. The visible stone or block is the smaller part of the job; the base and drainage are what determine whether it lasts.
